Making a Referral

What's a partnership with GOALS look like?

GOALS Staff love working with our Omaha Metropolitan Community schools to build relationships and partnerships. We understand that it takes a village to address the factors that contribute to absenteeism. GOALS works with students from all 11 public school districts in Douglas and Sarpy Counties. GOALS is best utilized after schools have implemented their own interventions and find that an intensive case-management approach would be helpful. Students from Kindergarten through twelfth grade are accepted.

Monthly Reports

GOALS Family Advocates provide monthly reports to our referring parties- providing updates, progress, and specific plans to address attendance. This includes providing copies of the monthly service plans to students and their families.

Attendance Success Plans

Students, their families, and the referring parties will receive a copy of the student's Attendance Success Plan upon case closure.  These plans detail how the student will maintain their attendance as well as a list of resources in the community for the family to utilize.

You will need to make a referral: 

​

  1.   Student info

  2.   Caregiver contact information 

  3.   History of absenteeism 

  4.   Agreement to participate from caregiver

  5.   10-15 minutes to complete the referral form
